Doctor Luo Dalun continues his patent-medicine series. Yesterday he walked through a Tong Ren Tang store and was struck by the sheer number of ready-made Chinese patent medicines. His point: if you truly master these formulas, you can probably handle most everyday illnesses yourself. Today’s subject is Fang Feng Tong Sheng San — originally a powder, now made into Fang Feng Tong Sheng Wan (pill), granules and other forms; what Tong Ren Tang sells is the pill. Why talk about this formula? Because it treats exterior-contraction illness — the common cold and flu.

Why discuss it now? — The weather is turning cold, and the epidemic is back

The reason for talking about exterior-contraction illness is that the knowledge is needed right now.

💬 Note from Doctor Luo: The weather is turning cool, and you can see the epidemic has surged into a second wave, right? Internationally, Germany has been adding more than ten thousand cases a day, and France fifty thousand a day — it has erupted again. Every time the temperature drops, it erupts again. These past days Kashgar in Xinjiang reported 137 new asymptomatic infections, and some experts link this to the region’s sharp temperature drop. So everyone should stay on high alert.

China’s containment has been excellent — by global standards it is an outstanding example — but ordinary people must not become complacent. The more you know about exterior-contraction illness, the better. It never hurts to learn more.

Who created this formula? — Liu Hejian, the “heat-clearing master” among the Four Great Masters of the Jin-Yuan

Who created Fang Feng Tong Sheng San? Liu Hejian (Liu Wansu).

💬 Note from Doctor Luo: Liu Hejian was one of the Four Great Masters of the Jin-Yuan dynasties. We have been discussing Li Dongyuan; Li Dongyuan and Liu Hejian were both among the Four Great Masters. Liu Hejian came earlier and was somewhat older. What did Liu Hejian emphasize? Clearing heat — he paid special attention to problems caused by heat evil in the body, so he placed great weight on clearing heat.

Why is Fang Feng Tong Sheng San so classic? Because it embodies an important method called treating both the exterior and the interior (biao li shuang jie) — this was a signature of Liu Hejian’s prescribing; a whole series of his formulas work this way.

Signature move: treating both exterior and interior

What does “treating both exterior and interior” mean? Start with how an exterior pathogen invades.

💬 Note from Doctor Luo: When a pathogen attacks, if it is cold evil, the first stage is exterior cold — the body surface is cold: sneezing, clear runny nose, chills all over, fever. That is the exterior-cold stage. If the exterior-cold stage is never resolved, and the person also carries heat inside — say they usually eat very well, rich and fatty food, with heavy phlegm-heat, or their internal fire is strong — then when the pathogen pushes inward, it readily does what we call transforming into heat upon entering the interior (ru li hua re).

Why does an inward-moving pathogen turn into heat? Because as it travels inward, the upright qi fights it inside the body — the battlefield is covered in smoke and wreckage — so the body raises its temperature to wage the war, and heat signs readily appear inside.

💬 Note from Doctor Luo: In this situation the exterior cold has not yet been resolved while interior heat is present — this is the exterior cold with interior heat pattern, also called cold wrapping fire (han bao huo). It is a state many people enter after catching an exterior contraction.

How does a cold turn into “cold wrapping fire”? — the trap of not being able to tell wind-cold from wind-heat

Once you reach the cold-wrapping-fire stage, using only warming methods or only cooling methods will no longer work.

💬 Note from Doctor Luo: Use warming herbs and you scatter the exterior cold — but what happens to the interior heat? It gets worse, much worse. Use cooling herbs and the exterior cold becomes unbearable. So at this stage neither a purely warming nor a purely cooling approach will do.

Many people fail to recognize the exterior-cold-with-interior-heat pattern, and then their cold becomes a puzzle:

💬 Note from Doctor Luo: If you insist on classifying every cold as wind-cold or wind-heat, then which is yours? Your nasal discharge is clear one moment and yellow the next; you feel chilled outside yet feverish inside; cold and heat are tangled together. Classify mechanically and you will not be able to treat your cold at all. Many friends — including TCM students — cannot work out whether they have wind-cold or wind-heat, follow the textbook, fail to treat it, and land in a dead end.

Liu Hejian’s exterior-interior double resolution solves exactly this problem: exterior cold with interior heat — I resolve both at once. Use warming herbs to scatter the exterior cold, and a group of cooling herbs to clear the interior heat, together. Once the body’s state is adjusted back to balance, the body itself will drive the pathogen out.

Formula layer 1: ma huang leads the charge, fang feng and jing jie help out

How does Fang Feng Tong Sheng San deploy its troops for double resolution? Look first at the group that scatters exterior cold.

💬 Note from Doctor Luo: The formula’s first herb is one that clears exterior cold, and it is necessarily warming: ma huang (ephedra) — as everyone knows, ma huang scatters cold, releases the exterior and induces sweating; it is a sweating agent. Two more herbs are sent to help ma huang scatter the exterior cold: fang feng (ledebouriella) and jing jie (schizonepeta). Both are wind-dispelling, cold-scattering herbs we use constantly in treating exterior contractions — they are a classic herb pair, almost always used together.

Formula layer 2: da huang and mang xiao — urgently descend to preserve yin, free the bowels first

With the exterior-cold herbs in place, Liu Hejian then sends a large contingent of heat-clearing herbs. By the time a case calls for Fang Feng Tong Sheng, the interior heat is generally severe — heat evil has accumulated, and heavy heat damages body fluids.

💬 Note from Doctor Luo: When body fluids run low, the bowels tend to block up. What do you need then? Urgently descend to preserve yin (ji xia cun yin) — free the bowels promptly. Once they are open, heat can no longer accumulate inside, and fluids stop being damaged. Otherwise you get a vicious cycle: the more heat, the more fluid damage; the more fluid damage, the more the bowels block; the more they block, the less heat can escape; and the more fluids are lost.

So freeing the bowels at this point is critical. The formula uses da huang (rhubarb) and mang xiao (mirabilite) — the heavy hitters for moving the bowels. Once the stool moves, the heat evil lodged in the stomach and intestines is drained away and the fluids are preserved.

Formula layer 3: hua shi and zhi zi — heat leaves with the urine

Next comes another contingent: hua shi (talc) and zhi zi (gardenia fruit).

💬 Note from Doctor Luo: hua shi pours heat out through the urine. zhi zi clears heart fire, frees the qi of the three burners and drains the fire of the three burners; it, too, is good at sending the heat evil of the three burners out with the urine. So da huang and mang xiao free the bowels to drain heat, while hua shi and zhi zi open the water passage so heat evil leaves with the urine.

Formula layer 4: shi gao, lian qiao and huang qin — clearing lung and stomach heat

Another group of heat-clearing herbs follows: sheng shi gao (raw gypsum), lian qiao (forsythia) and huang qin (scutellaria).

💬 Note from Doctor Luo: sheng shi gao, as everyone knows, drains heat and sends heat qi outward and through. lian qiao detoxifies. huang qin clears lung heat. All are detoxifying, heat-clearing herbs. Shi gao paired with lian qiao and huang qin can basically resolve lung and stomach heat — this is one of the formula’s finest points.

Formula layer 5: jie geng and bo he — the rescue squad for a sore throat

When interior heat is really heavy, the throat is bound to be uncomfortable — a person with interior heat who catches an exterior contraction usually develops a swollen, painful throat. The formula therefore includes jie geng (platycodon) and bo he (mint).

💬 Note from Doctor Luo: What does bo he do? It clears the throat and pushes heat evil outward. jie geng detoxifies — it resolves the toxin of the throat — and also frees lung qi and sends it upward. Both herbs serve several functions at once, and jie geng with bo he is a particularly apt pairing. I think these two are used superbly.

Formula layer 6: chuan xiong, dang gui and shao yao — why nourish the blood while scattering cold and clearing heat?

The formula gets even better below: it adds chuan xiong (ligusticum), dang gui (angelica) and shao yao (peony) — to nourish the blood, especially the liver blood. The ancients had a fascinating explanation for this.

💬 Note from Doctor Luo: The ancients said, “When wind causes trouble, the liver wood rules it” — whenever a wind pathogen arrives, whenever a wind-driven disorder appears, the liver wood is involved, so at such times you must nourish the liver.

Doctor Luo’s own explanation comes from another angle:

💬 Note from Doctor Luo: In every situation like this, the channels are bound to be blocked — the pathogen has arrived, heat evil is congested inside, cold evil sits on the body surface, and interior and exterior cannot communicate. Why can’t they communicate? Because the qi and blood in the channels are no longer flowing properly. I always use this analogy: imagine the river carries troop ships, and the river connects every sentry post on the defense line. If the river runs dry, the ships run aground — how do you move your soldiers? The human body is the same: the channels are that river, and what flows in them is qi and blood, the defensive and nutritive qi. If they cannot be transported, how can you defend against the enemy? So you must replenish the blood. This is an important idea in treating exterior-contraction illness.

That idea is already present in the very first formula of the Shang Han Lun (Treatise on Cold Damage): Gui Zhi Tang (Cinnamon Twig Decoction).

💬 Note from Doctor Luo: You may ask: Gui Zhi Tang contains no heat-clearing, toxin-removing herbs at all — how does it drive the pathogen out? Because it contains something that opens the channels: gui zhi (cinnamon twig) moves blood and frees the channels. And shao yao — some, like Xu Shuwei, held that the formula used chi shao (red peony) rather than bai shao (white peony); what does chi shao do? It moves blood and frees the channels. So Gui Zhi Tang already works by moving blood, freeing the channels and keeping the qi-blood passage running. Sadly, later generations forgot this — when treating exterior contractions they only thought of clearing the pathogen: how do I scatter cold, how do I clear heat? They never thought of changing the body’s own internal state. So why is Fang Feng Tong Sheng San a classic? Because its thinking is extraordinarily thorough.

Formula layer 7: gan cao and bai zhu — beyond nourishing blood, also strengthen the spleen

Is nourishing and supplementing the blood enough by itself? No — you must also strengthen the spleen.

💬 Note from Doctor Luo: The qi of the spleen and stomach is our root. When spleen qi is sufficient and spleen earth is strong, lung metal can grow strong in turn (cultivating earth to generate metal, pei tu sheng jin). So you must supplement the spleen. How? With gan cao (licorice) and bai zhu (atractylodes) — bai zhu is the dedicated spleen tonic; gan cao also detoxifies and harmonizes all the other herbs. Supplement the spleen fully, nourish the blood well, let spleen qi generate upright qi and qi and blood — fill the qi-blood passage and get it running — and only then scatter the exterior cold outside and clear heat inside.

So this formula is not only a double resolution of exterior and interior; it also dispels the pathogen while supporting upright qi (san xie fu zheng) — the logic is crystal clear, and it is a truly classic famous formula.

When to use it? — three signals of excess in both exterior and interior

When should you use Fang Feng Tong Sheng San (or the pill)? The core is four characters: biao li ju shi — excess in both the exterior and the interior.

💬 Note from Doctor Luo: When the exterior cold is heavy — severe chills — and the interior heat is also fierce, giving scanty and blocked urination and defecation (urine scanty and very yellow, stool dry and hesitant), bitter taste and dry throat — the mouth parched, the throat even painful. At such times the tongue is characteristic: the tongue body is red (the red showing through at the edges means heavy interior heat), the coating is yellow and greasy (very heavy interior heat), and the pulse is rapid. When an exterior-contraction illness sits in this state of cold outside and heat inside, this formula is the one to use.

When absolutely NOT to use it? — the truly deficient must stay away

One warning: who cannot use it?

💬 Note from Doctor Luo: People who are truly deficient. This formula is for excess in both exterior and interior — it is for when the pathogen evil is at its height. Some people are so deficient they pour sweat — they sweat at the slightest movement, they lie in bed too weak to move. Don’t give them Fang Feng Tong Sheng. Why not? The downward-draining, heat-clearing force of this formula would probably be more than they can bear. Their condition is not exterior-interior excess; it may be a deficiency pattern. You must distinguish clearly: use it only when the heat evil is at its full height.

Not just for colds: boils on the nose, carbuncles of the head and face, and red-eye all respond

Besides colds, the formula works for other diseases driven by very heavy interior heat with heat evil congesting the body — its main job is clearing heat; scattering the exterior cold is how it opens the body surface. It is especially good for skin diseases in which heavy interior heat surfaces on the body.

💬 Note from Doctor Luo: Here is an example. Some people get a big red boil on the nose — suddenly the nose turns red, with a large sore, a large lump on it. This is lung-stomach heat, especially of the spleen. Our nose corresponds to the spleen (in the middle burner): eat too richly, the heat cannot disperse, it accumulates and transforms into fire, and a large abscess or carbuncle forms on the nose. Fang Feng Tong Sheng works extremely well here — it clears heat and at the same time diffuses and releases lung qi, and the lung opens into the nose, so lung-stomach heat shows up on the nose. Other boils and carbuncles of the head and face — as long as they are caused by congested heat evil — also respond to Fang Feng Tong Sheng.

Eye diseases are the same — a sudden red, swollen, hot, painful eye, such as acute conjunctivitis, also responds to this formula.

💬 Note from Doctor Luo: What does it do? It distributes the heat evil downward and out of the body. So for all sorts of conditions with heat evil congested inside, Fang Feng Tong Sheng is effective, because its exterior-interior double-resolution thinking is very clear.

Carried to Japan as a “slimming pill”: what modern research discovered

Formulas like this one later reached Japan, where they became Kampo medicine — the Japanese began manufacturing it and researching further functions.

💬 Note from Doctor Luo: What did the Japanese research it for? Weight loss — and their studies found that not only does body weight drop, cholesterol and triglycerides fall beautifully too. So Japanese women use it to slim down. Their experts say Fang Feng Tong Sheng can expel metabolic waste and toxins through the digestive tract and the skin. They describe it as specially suited to the obese type of person who may be headed for stroke — the preventive effect is excellent, especially for people with big protruding bellies; the effect is even better there.

Modern research has also found it lowers blood viscosity, improves platelet aggregation, acts against arrhythmia, and lowers blood pressure.

💬 Note from Doctor Luo: So in Japan, modern research has given Fang Feng Tong Sheng many more therapeutic indications. These are things we should all pay attention to — our Chinese ancestors’ formulas have also won attention abroad; people there are studying new uses for them and proving the effects are excellent.

Summary: turn patent medicines into your “arsenal”
